My Labs Are Normal But I Feel Awful: What's Actually Going On With Your Hormones

You sit in the parking lot after the appointment, results in hand, every box inside the reference range. You should feel relieved. Instead you feel dismissed, because you know your body, and your body is telling you something is off. The exhaustion. The weight that showed up with nothing changed. The 3am wakeups with your brain fully online. And now a piece of paper that says everything is normal.
You are not losing your mind. But the conversation you have been having about your hormones probably needs to change.

"I have a hormone imbalance" is not actually the right question
"Hormone imbalance" is not a medical diagnosis.
It is not in any textbook, no single lab test can find it, and no specific treatment is prescribed for it. That is part of why doctors brush it off. It is like telling a mechanic "I think something is wrong with my car." The mechanic is not being difficult when they ask which part, what is it doing, and when does it happen.
Your body makes over 50 different hormones, and they all do completely different things.
The real questions are: which hormone, why is it off, and when in your cycle is it happening?
"Your labs are normal" does not mean everything is fine for you
Normal ranges are averages, not answers.
They are built from huge populations. They do not tell us where you personally feel your best, they do not capture how your hormones move across your cycle, and they do not account for how the ratio of one hormone to another might be the real issue.
Feeling terrible with normal labs does not mean you are imagining things. It means the questions need to get more specific.
"Estrogen dominance" is not what the wellness internet says it is
The feeling is real. The label is off.
Estrogen dominance is not an official diagnosis either, but it usually points to something specific: progesterone is low. Even when estrogen is completely normal on paper, if progesterone is not there to balance it, estrogen wins by default.
That ratio problem can look like heavy or irregular periods, bloating, breast tenderness, mood swings, poor sleep, and weight gain at the hips and thighs.
"Perimenopause would show up on your labs" is exactly backwards
In perimenopause, normal is a moving target.
Starting anywhere from your late 30s, estrogen can spike one month and crater the next, while progesterone declines as ovulation gets less consistent. Your doctor drew blood on one random Tuesday and may have caught a calm day.
A normal result does not mean you are not in perimenopause. The 3am wakeups, the out-of-nowhere anxiety, the palpitations, the joint aches, the brain fog, the mood changes that feel too big for your life: these are not character flaws. Estrogen and progesterone interact directly with your brain chemistry.
This is biology, and it deserves a real clinical conversation.
"PCOS is a period problem" undersells the whole condition
The root of PCOS is testosterone and insulin, not your uterus.
There is a newer name gaining traction, PMOS, polycystic metabolic ovary syndrome, because the syndrome is much bigger than just the ovaries. Excess androgens disrupt ovulation, and no ovulation means no progesterone, so estrogen goes unopposed by default. Meanwhile high insulin tells the ovaries to make even more testosterone, androgens worsen insulin resistance right back, a vicious cycle, and high insulin also lowers SHBG, so more testosterone runs free and active. If you chase estrogen when the real drivers are androgens and insulin, you are treating the wrong thing entirely.
Address the real targets and the downstream picture often starts to correct itself.
"Lifestyle advice is just fluff" gets the biochemistry wrong
Sleep, blood sugar, strength, and stress are hormone regulators, not wellness decor.
Most hormone regulation happens overnight, so 7 to 9 hours of sleep is a hormonal necessity, not a luxury. Blood sugar stability matters enormously, especially in PMOS, because insulin spikes directly amplify androgen production. Strength training helps your cells use insulin more efficiently. None of this means lifestyle fixes everything. Some people absolutely need medication, and there is no shame in that.
But lifestyle is biochemistry, and it belongs in the clinical conversation.

What is PMOS and how is it different from PCOS? PMOS, polycystic metabolic ovary syndrome, is a newer, more accurate name for PCOS. Same condition, better framing: the core problem is androgen excess plus insulin resistance, and it reaches far beyond the ovaries.
Why does this matter especially for South Asian women? South Asian women carry higher rates of insulin resistance and PCOS, often at "normal" weights and with "normal" labs, which makes specific questions about androgens, insulin, and SHBG even more important.
